How do I activate the Requires Preparation/ Approval option for holds on an ad-hoc basis?

You can manually select the Requires Preparation option on an ad-hoc basis for an item after the item has been checked in, from the patron Holds queue via Actions -> View/Edit.
After the option has been activated in the patron's account, the alert will also be displayed on the Check In screen and the Pickup Location report.
